If you have a scheduled appointment that you are not able to attend or no longer require, please call the clinic to cancel.
If you are experiencing symptoms of COVID and cannot come into the office because of those symptoms, please call us and we may be able to CHANGE YOUR IN PERSON APPOINTMENT TO a telephone consultation.
FAILURE TO CANCEL SCHEDULED APPOINTMENTS MAY RESULT IN A CHARGE TO THE PATIENT. AFTER FAILING TO ATTEND TWO SCHEDULED APPOINTMENTS, A FEE OF $50 WILL BE INVOICED TO THE PATIENT.
